About the Role:

As a Nurse Navigator, you will be responsible for providing guidance and support to patients and their families throughout their healthcare journey. You will work closely with healthcare providers to ensure that patients receive the best possible care and treatment. Your main goal will be to improve patient outcomes and satisfaction by providing personalized care and education. You will be an integral part of the healthcare team, helping to coordinate care and ensure that patients receive timely and appropriate interventions.

Minimum Qualifications:

  • Active nursing license (LPN or RN)
  • Associate's degree in nursing or related field
  • Minimum of 3 years of clinical experience
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team

Preferred Qualifications:

  • Certification in case management or navigation
  • Experience in patient education or coaching
  • Experience with electronic health records
  • Experience with administering health risk assessments
  • Experience working in a remote or virtual environment

Responsibilities:

  • Assess patient needs through health risk assessments
  • Provide education and support to patients and their families
  • Collaborate with healthcare providers to ensure continuity of care
