Responsibilities

  • Conduct thorough patient assessments, including mental health evaluations and medical histories.
  • Develop and implement individualized treatment plans based on patient needs.
  • Administer injections and manage medication regimens as required.
  • Provide telehealth services to patients, ensuring accessibility and convenience.
  • Collect and process specimens for laboratory testing as needed.
  • Collaborate with interdisciplinary teams to coordinate care for patients in assisted living facilities.
  • Monitor patient progress and adjust treatment plans accordingly.
  • Educate patients and families about mental health conditions, treatment options, and coping strategies.
  • Maintain accurate and confidential patient records in compliance with HIPAA standards.
  • Engage in REMS documentation as needed

Qualifications

  • Master’s degree in Nursing.
  • Active license in Illinois.
  • Board Certification in Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) is required.
  • Proficiency in tele-health platforms and virtual patient care.
  • Strong skills in patient assessment and diagnosis
  • Experience in psychiatric care, preferably in outpatient settings.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a team.
  • Compassionate, empathetic, and patient-centered approach to care.
  • Experience administering injections is preferred.
